SADA is excited to announce that we will be showcasing our new CarePak offering at HIMSS 2016 Conference & Exhibition with the Microsoft pavilion (Booth #3832). The Dynamics CRM based offering is designed to streamline and automate the scheduling and patient engagement process for healthcare organizations. The CarePak solution allows healthcare teams to stay on top of a patient’s care plan with an integrated calendaring system configurable to specific protocols. Administrators can assign tasks to care team members, track patient interactions or metrics, and create unique alarms to alert supervisors if action is needed.
“We designed CarePak to address challenges for healthcare teams around scheduling, follow up and data records to ensure that care members have all the information and tools they need to maintain a full 360-degree view of cases as well providing the best possible care before, during and after a patient visit,” stated Gregory Gant, Practice Lead for Dynamics CRM at SADA Systems. “By automating these administrative tasks in Dynamics CRM through the CarePak workflow, care members can focus on connecting and engaging with patients.”
The CarePak workflow utilizes Microsoft Dynamics CRM’s integration with Healthcare ERP systems to provide clinicians with the most accurate outpatient information as well as the ability to create unique alerts and track specific data points to stem notifications when a patient could be at risk. Automated touch points are created to connect and engage with patients through the discharge process as well as maintain a full 360-degree view of patient cases through a customized web portal.
The system comes pre-loaded with a generic care plan which is configurable to the organization’s specific guidelines and protocols. Care team members are assigned to tasks on a patient’s care plan via integrated calendaring, and are subsequently alerted as actions from the care plan are due.
Interactions with patients are tracked through the system which can include notes and specific metrics such as weight or any other relevant details. Alerts can be set to go off for a supervisor if any action is overdue by a certain amount, for example, if a patient’s weight breaks certain thresholds or the patient is a no-show for an appointment. This automated method allows healthcare organizations to streamline and automate scheduling and patient data pre- and post-discharge, ensuring greater accuracy and relevancy of patient engagement data and follow up.
HIMSS will be held Feb. 29 – March 4 at the Sands Expo Center, Las Vegas. Visit the Microsoft Pavilion to view a demo of SADA CarePak.
